If you have been told you have high blood pressure or diabetes, the dietary advice that follows is usually short and familiar: cut down on sugar. New research suggests that advice, while not wrong, may be starting in the wrong place.

The short answer appears to be yes — carbohydrate quality may deserve attention before sugar quantity. A study published on 11 August 2026 in npj Science of Food, led by Linjiang Wei and colleagues at Xiamen University, analysed UK Biobank adults with hypertension (34,179), diabetes (5,768), or both conditions together (3,772). The researchers compared two ways of judging a diet: how much sugar it contained, and what they called carbohydrate "matrix quality" — essentially whether the carbohydrates arrived intact, wrapped in fiber and bran, or stripped and refined. Matrix quality turned out to be the more consistent predictor of how fast the body was ageing.

How Scientists Measured Biological Ageing

Rather than relying on birthdays, the researchers used biological ageing markers built from blood chemistry — measures known as PhenoAge acceleration and a cardiometabolic marker burden score. These estimate whether your body looks older or younger than your actual age. Each standard increase in carbohydrate matrix quality was associated with 0.39 fewer years of PhenoAge acceleration in the hypertension group, 0.45 fewer in the diabetes group, and 0.53 fewer in those with both conditions. Notably, the benefit was largest in the group carrying both diagnoses — the people usually given the strictest sugar advice.

Why Low Sugar Alone Was Not Enough

The most striking finding was a comparison the researchers ran between four groups. People eating poor-quality carbohydrates but little sugar still came out biologically older than people eating high-quality carbohydrates with the same low sugar intake. In other words, cutting sugar while still living on refined flour, white rice, and heavily processed carbohydrates did not deliver the protection you might expect. Adding carbohydrate quality to a sugar-only model improved it more reliably than adding sugar figures to a quality-only model — a technical result with a simple message about which question to ask first.

The Kidney Findings

The team also tracked new cases of chronic kidney disease over time, and the same hierarchy held. Translated into everyday terms, the difference between the best and worst carbohydrate quality worked out to 0.8 percentage points of 10-year absolute risk in the hypertension group, 1.8 in the diabetes group, and 3.1 in those with both. Kidney damage is one of the quieter complications of long-standing diabetes and blood pressure problems, which makes a dietary lever this ordinary worth paying attention to.

What this research points to is not a low-carb diet but a better-structured one. These grains were the Indian default for generations before refined flour became convenient. Simple Ways to Improve Your Carb Quality

You do not need to count anything to act on this. Choose the version of a food that is closest to its whole form: atta over maida, hand-pounded or brown rice over polished, whole dal over the most processed options, and millet rotis a few times a week. Pair carbohydrates with a protein or fat source so the meal digests more slowly.
